We recommend you complete your 12 months journey at least as these Care Plans are for chronic conditions which develops over the years and require long term intervention to show up the results. However, it is entirely up to the customer to continue the same for entire 12 months duration.

Yes. HCL Healthcare offers you the same services even after you leave your current employer. However, from the day you leave your current employer and if the program was being paid by your employer, service will be charged directly from you, and it will not be cashless anymore thereafter.
